Can the welcome go to a DM instead of a channel?

Yes. Leave the channel out and it arrives as a direct message. Goodbyes need a channel, because Discord won't reliably let a bot message somebody who left.

Why didn't somebody get welcomed?

If your server makes people accept rules first, the welcome waits until they do. Accounts turned away by the account age gate aren't welcomed either.

How to set it up

Run /welcome set with your message. Give it a channel to post publicly, or leave the channel out and it arrives as a direct message instead.
You get a preview back straight away, so you can see exactly what a new member will see before anybody joins.
The welcome card is set up on the dashboard, under Welcome: switch it on, pick a background style or upload your own picture, and watch the live preview redraw with your own avatar as you type.
